Elevate 360 is an Out of Network Provider with insurance companies, meaning that we are able to provide each individual with the care that they deserve without insurance or 3rd party limitations. Each individual will receive 1 on 1 care with a Doctor of Physical Therapy for 1 hour long appointments. Payment is due at the beginning of the visit and a superbilll can be requested with diagnosis and treatment codes. This is on an as needed basis and will be the responsibility of the patient to submit to their insurance company for potential reimbursement.
There will be a $50 cancellation fee to anyone who cancels within 24 hours of their appointment. After a second cancellation within 24 hours of their appointment, the full appointment rate will be charged. This is to ensure that individuals waiting for an appointment can be contacted with advanced notice.

60 minutes
The first visit is a 1-on-1, hour long appointment which is spent looking into the cause of your pain and gaining a thorough medical history that could be relevant to your problem. At this visit, your therapist will explain all findings to you and give adequate treatment suggestions based upon these findings. She will establish a plan of care with you to address all aspects of your health and wellness and give recommendations for a timeline of future appointments.

Visits after the initial evaluation are considered follow up appointments. In these visits your Physical Therapist will use corrective exercises, manual therapy techniques and educational interventions related to patient specific needs found in the initial evaluation. Dry needling can also be a treatment option during a follow up visit.

Dry Needling + full treatment
Dry Needling only (once you’re an established patient)
Dry needling is a treatment option that can be used in conjunction with a full treatment or may be used independently. An initial evaluation must first be performed prior to receiving a dry needling treatment.

Sports Performance and Wellness visits are utilized when looking to optimize movement and prevent injury occurrence. Both types of visits are 60 minutes in length and include an evaluation to assess which type of program would be most beneficial.
